The answer so far
Italian product information describes Zadaxin, a thymosin alpha-1 medicine, as an adjunct to influenza vaccination in immunocompromised people. It is not a vaccine or a substitute for one. Studies have measured antibody responses in specific groups, but those measurements do not automatically establish fewer flu infections or apply to every peptide product.14

What is the medicine meant to do?
Vaccines introduce the immune system to a particular target, helping it build a response and memory. An adjunct is something used alongside another intervention with the aim of improving its effect. The Italian product information for Zadaxin describes it as an adjunct to influenza vaccination in immunocompromised people, whose immune responses can be affected by a condition or treatment. This is a named product and a specific national indication; it does not mean that Zadaxin is itself a vaccine, or that every product labelled thymosin alpha-1 has the same quality, evidence or authorisation.12
- Italian indicationZadaxin is an adjunct to influenza vaccination in immunocompromised people.
- 1989: older menA placebo-controlled study measured antibodies after an older seasonal vaccine.
- 2012: dialysisAn open-label pilot compared antibody responses with a specific H1N1 vaccine.
- What remains uncertainAn antibody result alone does not establish fewer clinical infections.
Why did researchers measure antibodies?
Antibodies are proteins made by the immune system that can recognise particular targets. Researchers can measure an antibody response after vaccination as one indication of immune activity. The measurement can be useful, but the amount of antibody alone is not a guarantee that a person will avoid influenza, and different tests measure different properties. A study of clinical protection would also need to observe relevant illness outcomes.25
What did the study in older men find?
A 1989 double-blind study randomly assigned 90 men aged 65–99 to thymosin alpha-1 or placebo alongside a seasonal flu vaccine. Blood samples from 85 men were usable for analysis. At six weeks, average antibody response was higher in the thymosin group, with an age subgroup contributing to that difference. The study tested an older vaccine and a particular population; the reported antibody finding does not establish how much the medicine prevents clinical influenza today.3
What happened in the dialysis study?
In a 2012 exploratory, open-label study, 99 people receiving haemodialysis were included in the safety population; 94 had at least one follow-up assessment. Participants were randomly assigned a particular pandemic H1N1 flu vaccine alone or with one of two thymosin alpha-1 regimens. Some antibody measurements were higher in the thymosin groups at day 21, while the study reported a greater decline in one antibody measure by day 42. The authors considered the early immune response encouraging and called for larger studies. This was not a direct demonstration of fewer flu infections.45
Why can assessments differ?
In a 2024 review for a US compounding advisory meeting, FDA scientists noted the small group sizes, differences in antibody levels before vaccination, incomplete information about additional vaccine doses and differing results by time and test. They considered the evidence they reviewed insufficient to establish thymosin alpha-1 as an effective flu-vaccine adjunct. That is a US scientific assessment of the available evidence, not a cancellation of the Italian product indication. Neither assessment means that all countries regulate the same product in the same way.51
What should a reader take away?
A person’s vaccination plan depends on their health, medicines, age and location. Ask a clinician about the relevant vaccine and the exact authorised products; do not substitute an online peptide for vaccination or change prescribed treatment independently. The Italian Zadaxin information also lists pregnancy and breastfeeding as contraindications and calls for individual assessment in autoimmune disease. For the broader distinction between immune markers and health outcomes, read what immune-support claims mean; for the molecule itself, return to the thymosin alpha-1 profile.12
